Becoming a Patient
Patients will pay a flat annual fee or flat monthly fee to become a patient of Dr. Jacobs’ concierge practice.
- under 45 yrs -$1600/year
- 45-55- $2000/year
- over 55 $2400/year
Complete yearly physical with labs and ECG included in the membership.
For Each Additional Visit
If you want to see the doctor, an additional visit fee will apply.

- Housecalls extra
*additional costs may apply for additional services, including joint injections, dermatologic procedures, medication injections, and other in office procedures.
